Adults aged 18 or over who currently smoke or vape
The only clinical threshold. Nothing else needs checking before you mention it.
Patients who have declined or disengaged from the local authority service
A second option to offer when the first one has already been refused.
Patients in areas with limited local authority provision
Where commissioned provision is thin, closed to new starts, or waiting listed.
Patients who have tried licensed NRT without success
Previous attempts do not exclude anyone from walking into a pharmacy.
How signposting works
Three steps, and none of them are yours
Your team mentions local pharmacy support
In an ordinary consultation, a receptionist, nurse, GP or dentist mentions that nearby pharmacies offer support, and hands over a card.
The patient finds their nearest pharmacy
The card carries a QR code. The patient enters their postcode and sees participating pharmacies near them.
The patient goes in on their own
They present at the pharmacy counter and speak to the pharmacist. Nothing comes back to your practice.
Signposting, not referral.
No patient record moves between your practice and the pharmacy. There is no form, no system integration, no onward task, and no clinical liability for your practice. The patient chooses whether to go.

Who the patient speaks to: a GPhC pharmacist and team
Every participating pharmacy is led by a GPhC-registered pharmacist, with a trained pharmacy team supporting the conversation on the high street.
The pharmacist discusses the full range of options available, including licensed nicotine replacement therapy and the local authority stop smoking service where one operates.
